Alinea Performance is committed to maintaining the privacy of your protected health information (PHI). We are required by law to maintain the confidentiality of your health information.           Your Rights Regarding Your PHI<\/h2>

          1. Right to Inspect and Copy<\/h3>

          You have the right to inspect and copy your PHI that we maintain. To do so, you must submit a written request. We may charge a fee for the costs of copying, mailing, or other associated supplies.<\/p>

          2. Right to Amend<\/h3>

          You have the right to request an amendment of your PHI if you believe it is incorrect or incomplete. Your request must be in writing and include a reason that supports your request.<\/p>

          3. Right to an Accounting of Disclosures<\/h3>

          You have the right to request an accounting of disclosures of your PHI made by us. Your request must be in writing.<\/p>

          4. Right to Request Restrictions<\/h3>

          You have the right to request a restriction or limitation on the PHI we use or disclose about you. We are not required to agree to your request except when you request that we not disclose information to your health plan about services for which you paid out-of-pocket in full.<\/p>

          5. Right to Request Confidential Communications<\/h3>

          You have the right to request that we communicate with you about medical matters in a certain way or at a certain location.<\/p>

          6. Right to a Paper Copy of This Notice<\/h3>

          You have the right to a paper copy of this notice. You may ask us to give you a copy at any time.<\/p>

          Changes to This Notice<\/h2>

          We reserve the right to change this notice and make the new notice apply to PHI we already have as well as any information we receive in the future. We will post a copy of our current notice in our facility and on our website. The notice will contain the effective date.<\/p>

          If you believe your privacy rights have been violated, you may file a complaint with our practice or with the Secretary of the Department of Health and Human Services. To file a complaint with our practice, contact:<\/p>

If you provide us authorization to use or disclose PHI about you, you may revoke that authorization, in writing, at any time. If you revoke your authorization, we will no longer use or disclose PHI about you for the reasons covered by your authorization.
