In most cases, no. Most states allows for "Direct Access," meaning you can come directly to a physical therapist for an evaluation and treatment without seeing a doctor first. This gets you on the road to recovery faster. However, if you are using insurance there are some specific plans that do have requirements regarding signatures or referrals. If your insurance has specific requirements, we will let you know before your first visit. (Medicare does NOT require a referral to get started with therapy).

Male pelvic pain can come from pelvic floor muscle tension, nerve irritation (pudendal, genitofemoral, ilioinguinal), postural or lifting mechanics that overload the pelvic floor, prior prostate or abdominal procedures, chronic stress, tight hips or low back, or irritation of tissues surrounding the bladder, testes, or perineum. A pelvic floor physical therapist’s job is to identify your specific driver so treatment can be targeted and effective.
Pelvic pain is more concerning if you experience burning or stabbing pain in the perineum, scrotal/testicular pain, pain with sitting, urinary retention, numbness, bowel/bladder changes, blood in the urine, or pain that worsens rapidly. Absolutely. Pelvic floor PT is one of the most effective treatments for conditions like chronic prostatitis/chronic pelvic pain syndrome (CPPS), pudendal neuralgia, perineal pain, saddle pain, testicular pain, and pain with sitting. Treatment reduces nerve irritation, calms overactive muscles, and restores healthy movement patterns.
Without treatment, pelvic pain can worsen or spread into the low back, groin, testicles, or abdomen.
